East Cornwall Primary Care Network (“ECPCN”) provides a Frailty Home Visiting (“FHV”) service to support patients and Practices to manage the care of house bound, frail adults over the age of 18 years, to improve health outcomes and avoid unnecessary re-admission to Hospital.
The FHV team aims to ensure effective integrated working with all community providers to avoid duplication and streamline care provision.
Service provision:
The service provides the following services:
- Diabetic foot checks;
- Post Hospital discharge visits;
- Coordination of annual health checks for frail patients and the coordination of community services;
- Learning disability checks;
- Ear checks; and
- Welfare checks.
