Primary Care Network of the year
Primary Care Networks were introduced into the NHS around 2019 and are a group of General Practices focused around the local populations they serve.
For us in East Cornwall this means understanding our local population health and social care needs and ensuring that there are services and support functions that can help to improve your lives.
Typically this will include a wider healthcare team as well as linking with local services including Social Care, Schools, Ambulance, Police & Fire services whilst maximising the use of our community assets including sports and social centres.
